Care in a Facility
If a patient calls home a nursing center or assisted living facility and their physician has determined they have six months or less to live, Homestead will coordinate hospice services with the contracted facility. Understanding and acknowledging the importance of communication and care collaboration between family members and facility staff are key elements of hospice care. Homestead’s professional team will communicate after each visit directly with the family and facility to keep everyone updated on the patient's status.
If the patient and family has any reason to have their loved one in an inpatient hospice unit, Homestead will provide the best care possible in a contracted facility to ensure all comfort measures.
