FACILITY COLLABORATION

Homestead Hospice recognizes that it is a privilege to be invited into your facility to provide care to your residents. We do not take this responsibility lightly. Homestead Hospice is not just another vendor in your building. We are a resource to you and will collaborate with your team in the care of your residents.

After consulting with Administrators, Directors of Nursing, Social Workers and Activity Directors in many facilities, we developed a comprehensive program that is tailored to your needs.

This program includes education, activities, spiritual connections, care collaboration, teamwork and communication to ensure best practices for both of our teams as we work together to care for your residents. We have designed a program, backed by processes, policies and tools to ensure that we deliver what we promise.

Service Oriented

  • Respond within one hour of your referral
  • Dedicated to serving patient needs
  • Nurse Aide visits as many times per week as needed
  • Watch List: If patient is declining in health but not ready for continuous care, the Nurse calls the family every shift to check on the patient
  • Passionate about taking care of the entire family's medical, spiritual, emotional and financial needs
  • Focused on community education

Proactive Care Offerings

  • Continuous care
  • Customer Service Nurse calls patients, families and/or facilities all day to ensure we are meeting our service goals and your expectations
  • Lead CNA supervises and schedules aides and filles in if necessary
  • On-call Nurses are backed by administrative call team of managers each night to ensure someone always visits the patient when needed
  • Support groups and memorial services offered to all of our active facilities
  • Homestead Hospice Medical Director visits each of our patients monthly
  • We treat symptoms, when needed, to provide comfort
  • We will give antibiotics to treat and infection
  • We will treat a wound to heal it

Facility Communication Program

Prior to working with your facility, our team reviews protocols (ours and yours) with your staff. We have developed specific tools to ensure communication between all staff members. Once protocols are established and logged, we then implement the use of our Communication Binder.

Facility Education Program

We offer many in-services to assist you in the education of your staff. If you have a need that is not on the list, just let us know what you need and we will tailor an in-service for your staff.

Facility Activity Program

Homestead Hospice offers a wide variety of recreational activities for your staff, residents and families. Call our Community Relations Manager for a full list or to request a special activity.

Facility Social Service Connections Program

We offer memorial services, staff support groups, family night education meetings, family support groups, social worker connections to families, facilty garden enhancement and holiday giving tree. Call our Community Relations Manager for more details.
