PERSONAL PHYSICIAN

The patient's personal physician is an integral part of the hospice care plan. We always include the patient's Personal Care Physician when there is one available. He/she will assist in all decision making for the care of the patient and will always be informed of any new paths taken by Homestead Hospice to better relieve the patient's symptoms and pain.
